TREATMENT OF ELBOW OSTEOARTHROSIS

Major advances in the recognition and treatment of elbow osteoarthritis were made in the last quarter of thecentury. Although primary elbow osteoarthritis is relatively rare, a better understanding of the anatomy of theelbow and the pathologic changes caused by osteoarthritis has led to a greater recognition of the disease. Thevolume of available information regarding elbow osteoarthritis has paralleled this increased awareness as evidenced by the increase in reports on this topic in the literature and increased interest in courses that teach elbow arthroscopy. Primary osteoarthritis of the elbow joint is characterized by painful stiffness, mechanical symptomsand the appearance of hypertrophic osteophytes. Possible to maintain the height of the joint space, which cancount on the good results of conservative treatment and nonprosthetic arthroplasty. Osteoarthritis of the elbowjoint is mainly prevalent in middle-aged men engaged in heavy physical labor. Main options for primary surgicaltreatment -open or arthroscopic release capsules and removal of osteophytes causing impingement. Advances inarthroscopic and open surgical approaches and improvements in prosthetic design have led to a high rate of clinically successful treatment of osteoarthritis of the elbow. Total elbow arthroplasty is rarely indicated in primaryosteoarthritis. This technique should be used in elderly patients with low requirements for which other treatments were unsuccessful. Overview: Elbow osteoarthritis is a disabling condition because of pain and loss of motion. It affects primarilymiddle-aged men engaged in strenuous manual activity. The best treatment option involves capsular release andremoval of impinging osteophytes. Historically, this has been done through open incisions and joint exposure. The use of arthroscopy has been an important improvement in our ability to address the problem through a minimally invasive approach and short-term and mid-term results are promising. Arthroplasty should be reservedfor older, sedentary patients for whom all other options failed.
