Orthotransaxillary variant of videothoracoscopic Access (topography-anatomic substantiation of surgical technology)
In this work we suggest a new method of orthotransaxillary endoscopic access on the base of anatomic experimental research. The suggested access allows optimizing interventions on lungs and mediastinal organs, reducing their traumatism and cosmetic defect, lowering the quantity of perioperative and postoperative complications. On the base of morphometric, stereometric and dummy experiments a complex of parameters, necessary for the estimation of endoscopic access were worked out, optimal parameter points were specified. A scheme of thoracoports setting and the complex of inner and outer anatomic landmarks were suggested for identifying the localization of intervention objects during videothoracoscopic operations taking into account a somatotype and patient's age.
