Reflection trends of perinatal affection of the central nervous system syndrome in the complaints of mothers in their interactions with pediatricians

We present the results of exploratory study, which aims at developing a model of early detection and control of the risks of deviation of somatopsychic development in children with perinatal affections of the central nervous system. They are applicable in the context of inter-agency support for families with children. 107 term infants with brain damage and their mothers have made the study sample. The group was divided in accordance with the dominant symptom perinatal affections of the central nervous system. We focus on the content of parents' messages to a doctor about their children. Correlating them with the chronology of the dynamics of clinical manifestations of the objective symptom of perinatal affections of the central nervous system, we found a tendency of forming a style by a mother in caring for her child's health. We have identified the core of the universal semantic component of parent complaints. It reflected mainly those signs of perinatal affections of the central nervous system that most clearly deviated from the mother's standard representations of the normal functioning of the child's body. We found that the general thrust of the requested pediatric care for more than an objective status of the child, determined the susceptibility of women to certain symptoms. Many women are excessively focused on the somatic-visceral component of symptoms underestimating dysfunctions of regulatory nature in their children. Thus the mother's perception of the status of children included both hypo- and hypernosognosic elements. We have described the mechanisms of formation of a vicious circle in the chain of constitutional-biological and socio-psychological factors of ontogenesis of children with PPTSNS due to their special needs for care. A key element of these mechanisms is the deformation of the communicative field in the mother-infant dyad. This happens because of asthenia in self-regulatory capacity of the mother. At the same time the focus of care shifts from significant emotional, social, and psychological needs of the child to the care of specific aspects of biological functions. We offered to study the possibility of early detection of deformation markers in dyadic relationship of a mother and a child with perinatal affections of the central nervous system from the standpoint of pediatrician and neurologist. We emphasize the importance of quality of interaction in a dyad mother-physician to prevent the depletion of a self-regulatory parental potential. Preservation of this potential we see as a key factor for favorable development of a child in terms of his or her constitutionally-biological attenuation. Determination of the specificity of the perception by the mother of clinical manifestations of symptoms of perinatal affections of the central nervous system is impossible without her trusting relationship with her doctor. We have also argued the necessity and appropriateness to involve a clinical psychologist at early stages in a multifunctional team to support families with children with PPTSNS. This will allow performing a primary prevention of deformation of ontogenetic significant parameters in social environment of children with perinatal affections of the central nervous system.
