Differentiation mechanisms of medical care consumption in Russia: a methodological turn in contemporary sociological research
The article deals with the specific features of the medical care consumption phenomenon in the sociological discourse. The author focuses on theoretical and methodological backgrounds of a sociological understanding of medical care consumption strategies, and determines differentiation mechanisms of medical care consumption in the context of transformation of the healthcare institution in Russia today. The aim of the article is to create a theoretical approach to a complex assessment of the healthcare system accessibility. The novelty of this endeavor consists in the application of the sociological constructivism methodology to a consumer's behavior study in the health sphere. The possibility of integrating economic, organizational and socio-cultural dimensions for a better sociological understanding of inequality factors in healthcare is argued. The concept of medical care consumption is analyzed as a dynamic interaction of different variables, the interaction of actors in the medical system in a changing social context. Special attention is given to the analysis of behavioral and structural dimensions of medical care accessibility. Despite considerable efforts by the health authorities to promote legislative and regulatory innovations, achievements in this field are minor. The limitation of the modern concepts of differentiation in healthcare is connected with the reduction of social inequality to objective geographical and economic factors, although it should rather be about the intersection of external and internal barriers of inequality. The study of the differentiation in the sphere of medical care consumption is dominated by two approaches: personality and situational. Currently, there is a tendency of overcoming the cognitive one-sidedness of personality and situational approaches. In line with this trend, the social inequality of consumers is not regarded as the result of influence of the social situation or individual traits, social values, attitudes and cultural predispositions. The social differentiation of medical care consumption is seen primarily as a result of people's interpretation of the social situation of health problems and "attributing" values and meanings to it in the socio-cultural context of the health care system.
